SOAP notes + records
SOAP notes for typed, dictated, templated, and handwritten charting
A veterinarian can type, dictate, use clinic templates, or write on paper while keeping the patient chart close at hand.
The beta includes patient charts, SOAP drafts, browser dictation, handwritten-note uploads, and storage of the original file. Creating a draft from handwriting requires a connected vision service.
Beta capabilities
What your clinic can test
Resume an autosaved draft with the patient’s problems, medications, allergies, results, and recent visits nearby.
Keep a photo or scan of the handwritten page with the record.
Compare extracted text with the original before adding it to the SOAP note or signing it.
Step by step
How staff complete the work
- 01
Open the note beside the chart
Open the note beside problems, medications, allergies, vaccinations, vitals, diagnostics, and recent activity.
- 02
Chart the way the veterinarian prefers
Type, use clinic templates and phrases, dictate in a supported browser, or upload a photographed or scanned handwritten SOAP page.
- 03
Review the note before signing
Edit the draft, confirm structured details, and sign when it is ready. Signed SOAP content stays locked, with later corrections appended and attributed.

Practical answers
Questions clinic teams often ask
Can a veterinarian keep writing SOAP notes on paper?
Yes. A clinic can print a SOAP template, write on it, and upload a photo or scan. vetverve retains the original alongside the note rather than treating paper as disposable.
Does handwriting transcription update the record automatically?
No. When an external vision service is connected, it creates editable text for review. A clinician checks and adds that text to the note, and vetverve never signs the note automatically.
What happens after a SOAP note is signed?
The signed SOAP body becomes read-only. Corrections are appended with the author, reason, and time so the clinical history remains traceable.
Does dictation require another service?
The beta build uses supported browser speech recognition. Availability and recognition quality depend on the browser and device, and the clinician reviews the text before signing.
